What We Do
The Christian Care Rehabilitation team bridges the gap between hospital and home. We help you get stronger, so you can return to your previous function and transfer back to home. Staff therapists and nurses provide one on one care as you recover froma medical or surgical procedure.
Helping You Understand
Your transitional care plan begins when you see a therapist in the first 24 hours of receiving orders from your doctor for a rehabilitation assessment. The assessment allows us to provide a personalized rehabilitation plan just for you. We help you get stronger to transition back to home and the community as quickly as possible with the best outcome during your stay with us. We even visit your home to assess your home equipment and safety needs before your return home. We also consult with you to determine if you require assistance at home.
